Guidelines for Substitute Oklahoma Tax Forms:
Request for Approval for Substitute Forms

Complete your Letter of Intent and mail it to the Oklahoma Tax Commission. We will file your letter and register your identification code. This identification code is of your choosing, and must appear in the lower left corner on all of your substitute forms.  If your code is like or too similar to another vendor, we will notify you so you can choose another code.  You may use your NACTP vendor code if you wish.

Once you have completed your substitute form, you must ensure it is clearly marked as a draft and send two (2) originals to the Oklahoma Tax Commission. A cover letter listing the form numbers submitted, name of company submitting, an area to indicate if each form submitted us approved, in need of change, or is rejected, and  phone and fax numbers of contact/representative must accompany each submission. We will review your submissions and inform you of approval or changes required within 10 business days via  fax. If changes are needed you may be required to re-submit the forms until you have written approval.

Note: The Oklahoma Tax Commission does not review or approve the logic of specific software programs or confirm the calculations entered on substitute tax forms output from software programs.  The accuracy of software programs is the responsibility of the software developer, distributor, or user.
