MV: Active Duty Military Personnel Registration

Active duty military personnel who are either residents of, or stationed in, Oklahoma are entitled to an annual registration fee of $21.00.

A completed U.S. Armed Forces Affidavit (Form 779 – available for downloading from the Forms section of this site), attested to by an officer, must be submitted, along with insurance verification form, $21.00 registration fee and $1.50 insurance verification fee.
